The pulley fork is a common part that is damaged when you have an extension spring break. Also if your extension springs have been worn out and stretched out over time, it is common for the cables to come off of the pulley leaving the cable to cut into the pulley fork and the 3/8′ bolt eventually fraying the cable or cutting through the bolt of the the pulley fork. If the old pulley fork is bent, it is a good idea to replace it. Anytime metal bends it is fatigued and has stress fractures once it is bent back, it is better to be on the safe side.
